In the game of love the two partners should care for each other and learn about each other - hopes, dreams, fears, likes, dislikes, problems, etc. This narrator doesn't even care about his girlfriend's emotional problems and illness. He doesn't seem to know a thing about relationships. He doesn't care about her problems; it's just her pootang that he's concerned about. Therefore, contrary to the title of the song, the narrator of the song is the one who's got issues.
